The Social Worker fosters positive and professional interpersonal relationships with patients/families, physicians, hospital staff, community agency representatives and visitors and and provides comprehensive psychosocial assessment for identified patients and based on findings, facilitates interventions. Effectively uses interpersonal communication skills to resolve conflicts and promote healing environment.
More specifically this role provides a variety of essential services to HIV+ and HIV- adolescent and young adult patients and their families, including family-centered case management to those referred for care: intakes, assessment, support, and education. Identifies patient/family needs, strengths, satisfaction, and access to community-based resources. Collaborates with HIV-affected families to develop coordinated plans of care. Provides insurance counseling and assistance to apply for benefits. Makes referrals and linkages to appropriate medical care, mental health, educational opportunities and support services that reduce barriers to accessing care and aid medication adherence. Documents interventions and progress made in treatment plans and monitors and documents outcomes for referrals. Participates in multidisciplinary team meetings, case conferences, and development/implementation of peer support groups. Submits data on services provided and participates in program evaluations.
